Name, Address (City/State/Zip), Day phone, Cell Phone, Dates you want to attend, and whether or not you have studied at the NJ Center for Mime to yasshakoshima@verizon.net.
Email application and full payment or deposit must be received by October 26 (or October 15 if you choose the Trick or Treat option).
If you choose to pay a deposit, the remainder must be received on or before the first day of class.
