Send an email application including the following information:
Your Name, Address (City/State/Zip), Day phone, Cell Phone, Dates you want to attend, and whether or not you have studied at the NJ Center for Mime to yasshakoshima@verizon.net.

Email application and full payment or deposit must be received by August 10.
If you choose to pay a deposit, the remainder must be received on or before the first day of class.

$230 - Aug. 15, 16, 21, 22 (2 weekends; Full payment)
$125 - Aug 15 & 16 (Weekend 1; Full payment)
$125 - Aug 21 & 22 (Weekend 2; Full payment)
$50 - Deposit by Aug 10
2 weekend remainder: Pay by 1st class - $180
1 weekend remainder: Pay by 1st class - $75

Mail with a check for the deposit or full amount to:

Application and deposit must be received by August 10, 2010
